Blue Mountains Community Care Services Guide

Blue Mountains City Council has produced the Community Care Services Guide, a booklet that lists a number of services available throughout the Blue Mountains that can assist people who are frail-aged or living with a disability and their carers to live more independently.

The Community Care Services Guide has been recently updated as of March 2012.

Services in this booklet include Meals on Wheels, Respite Care and Home Help.

Provides meals on wheels, centre based meals and/or frozen food service to frail aged and younger people with disabilities, mental health problems and their carers. Has village restaurants at Lawson, Katoomba, Blackheath, Blaxland and Springwood.

Blue Mountains Home Modification & Maintenance Service

Blue Mountains Home Modification and Maintenance Service (HMM) is a Home and Community Care (HACC) program which is managed by Katoomba Neighbourhood Centre.
The service carries out modification and maintenance to enable people who are frail-aged or who have a disability to go on living in their own homes for as long as possible, rather than moving into nursing homes, hostels and other such facilities prematurely.

Blue Mountains Palliative Support Service is a free service that provides volunteers to support people who are terminally ill in their home, in local hospitals or (for limited periods) in aged care facilities. The service recruits, trains and supports volunteers.

Commonwealth Respite and Carelink Centres are information centres for older people, people with disabilities and those who provide care and services. Centres provide free and confidential information on community aged care, disability and other support services available locally, interstate or anywhere within Australia.

The Aged Care Rights Service (TARS)

1800 424 079 Provides free telephone legal information to older people and older people living in hostels, retirement villages and nursing homes. Blue Mountains City Council’s Aged and Disability Officer ((02) 4780 5546) can provide information about local disability and aged services.
